How to Register through Online registrations

How to Register Applications sent by mail

1. Obtain a registration form.

Contact your local Alzheimer Society and ask for a Safely Home registration form. Or you can download the form here.

2. Fill out the registration form.

Be sure to fill out the form completely, sign and date it.

3. Send in the completed form and cheque.

Make your $35 cheque payable to "Alzheimer Society of Canada," then mail your form and cheque, or take them to your local Alzheimer Society office or the national office of the Alzheimer Society of Canada.
